First Visit Information
We are so glad that you chose or were referred to Dr. Smith's Eye Care for your eye health needs and would like to make your first visit as easy and convenient as possible. Below is a comprehensive description of what to expect and bring on your first visit and what to expect on subsequent return visits.

When To Arrive
- You need to arrive 15 minutes early for your initial appointment in order to complete new patient registration
- A legal guardian must accompany any minor to their first appointment to complete registration
Check-In
When you first arrive at our office for a visit, please check in at the front desk. For further convenience, we have kiosks available for electronic registration at your request. Using the kiosk eliminates the need to fill out forms EVERY time you visit our office! Please bring the following items with you (as applicable):
- Vision AND Health Insurance cards of patient
- List of current medications
- Photo Identification of the Responsible Party (self or parent/guardian)
- Any referral form(s) that patient has received from primary care physician, ER, specialist, etc.
- Workers Compensation claim information (if applicable)
Co-Pays and Other Payments
- Your co-payment is required at the time of service
- We accept Care Credit, Visa, MasterCard, American Express, Cash, and HSA cards
Insurance Policies
- Are you aware of what your insurance covers? We cannot bill your insurance company unless you give us your insurance information. Your insurance policy is a contract between you and your insurance company; we are not a party to that contract.
- We will attempt to file with the insurance information provided, however, some or all services and material provided to you may not be covered as “reasonable and necessary” by Medicaid, Medicare, and/or other insurance providers. In the event that your insurance provider does not cover services rendered, the balance is your responsibility and our office will provide instructions on how to proceed.
- Regarding insurance plans where we are a participating provider, all co-pays and deductibles are due the day of service. In the event that your insurance coverage changes to a plan where we are not participating providers, you are responsible for the charges.
